Chapter 1 Autoimmune Disease: A Bird’s Eye View
Mistaken Identity
When you suffer from an autoimmune disease, it basically means your body is being attacked by your own immune system, the function of which is supposed to be to protect your body from viruses, bacteria, and other intruders. Affecting over 23 million Americans, this case of mistaken identity manifests itself in over a hundred different autoimmune conditions.
● Some of these diseases, as in the case of Hashimoto’s thyroiditis, involve one organ, while other diseases, like lupus, attack just about any organ/tissue.
● In the early stages of an autoimmune condition, symptoms like joint pain and fatigue usually resemble those that describe other health issues, which makes it challenging to make a diagnosis.
● While some autoimmune diseases are temporary, a lot of them can be lifelong, the reason sufferers may refer to their condition as an invisible disability. They face significant health problems, but they look okay on the outside.
Delicate Balance
Your immune system has the important task of protecting you from foreign substances, bacteria, viruses, and cancer cells, and it does the job skillfully with balance. On one hand, if your immune system is underactive, it is unable to mount good immune responses, which means that the most minor infections could be fatal. On the other hand, if your immune system gives an overactive immune response, which is the case with autoimmune conditions, it leads to illness and potential death.
Rising to the Occasion
Normally, you can rely on your immune system to rise to the occasion when, for instance, a virus invades your body. Immune cells, including lymphocytes, will be rushing to the area being attacked by the virus and create inflammation. The T lymphocytes have their part in destroying any kind of pathogen (innate immune response). Meanwhile, B lymphocytes are involved in producing antibodies that will target the specific threat (learned immune response). Your immune system generally does not fight against your body’s own cells, what with the presence of T helper cells and other regulatory steps that work on preventing autoimmunity from taking place. Still, it could still happen.
Reaction Creation
An autoimmune reaction comes about in a number of different mechanisms, including the following:
A microbe/foreign substance resembles your body - In the case of rheumatic fever, the proteins in streptococcal bacteria (group A) resemble the proteins found in the heart muscle, causing the antibodies to turn their attention to, and attack the heart.
Normal body cells undergo alterations – For instance, a virus enters the body and alters a body cell, causing the immune system to identify that cell as non-self

Antibody (B cells)-producing immune cells malfunction, causing them to produce abnormal antibodies that attack the body’s normal cells.
A substance in your body, like the fluid within your eye, which is usually not within sight of your immune system, finds its way into your bloodstream (like in the case of trauma).
Telling the Difference
Developing autoimmunity does not automatically mean a person has an autoimmune disease. For instance, your body does manufacture autoantibodies, antibodies against itself that help in clearing away post-infection debris. In the case of autoimmune disease however, the reaction results in inflammation and damage of tissues.
To Each Their Own
Autoimmune conditions come in a wide range and, together, can potentially affect the tissues in just about any part of the body. They can be classified according to the organ they primarily affect (organ-specific autoimmune diseases) or according to the multiple tissues/organs they have an effect on (systemic autoimmune diseases). The latter can affect the skin, muscles, joints, blood vessels, and endocrine glands, and their effects can be felt throughout the patient’s body.
Organ-specific autoimmune conditions include:
Autoimmune thyroid disease – Although autoimmune thyroid disease is an extremely common condition, it is believed to be seriously under-diagnosed. A person’s body may produce autoantibodies in response to a damaged thyroid tissue and an underactive thyroid (hypothyroidism), which is the case with Hashimoto’s thyroiditis, or as a result of thyroid tissue stimulation and an overactive thyroid (hyperthyroidism), which is what happens with Graves’ disease. These two conditions both involve symptoms that may rapidly take place or gradually happen over a certain period.
● Hypothyroidism may manifest itself in symptoms such as fatigue, hair loss, weight gain, and constipation. A person suffering from this condition undergoes treatment involving thyroid replacement medication, which will be a lifelong commitment.
● Sufferers of hyperthyroidism usually exhibit anxiety, nervousness, heat intolerance, and sweating. Treating this disease involves the use of antithyroid drugs, radioactive iodine therapy (for thyroid gland destruction), or surgery.
Psoriasis – A person suffering from psoriasis has an immune system that mistakenly sends the wrong message to the skin cells, signaling them to grow at a rapid rate. The most common form of psoriasis, which comes in several forms, is plaque psoriasis, wherein raised red patches that are usually itchy appear on the scalp, lower back, elbows, and knees. The type as well as severity of the condition is taken into consideration when looking into treatment options. Psoriasis patients need to have themselves screened for psoriatic arthritis, another autoimmune condition related to psoriasis.
Guillain-Barré syndrome – This autoimmune disease results when the support cells lining the nerves are attacked by autoantibodies. Usually occurring after an infection caused by a virus, it is suggested that Guillain-Barré syndrome comes about because certain parts of the pathogen resemble portions of the patient’s nervous system. A person suffering from it typically starts feeling weak and experiences sensation in the hands and feet. As the disease
